What FreebieAgent noticed
This is a legitimate sweepstakes from Keebler (owned by Ferrero, a major food company), tied to their Halloween Spookie cookies. The official rules link is present, and you're choosing between regional zoo prizes or a grand prize tier, which adds some real variety to what you could win. The catch: you'll need to consent to marketing emails from Ferrero brands, though text messages are optional. The form asks for your birth date and favorite retailer, likely for demographic tracking and targeting. Keebler is a trusted brand, so this isn't a scam, but expect post-entry marketing.
You fill these in yourself on the brand's site.
